Welcome to our practice! To help ensure the best possible experience for all our patients, please take a moment to review our office policies. 

Appointments

We value your time and strive to run on schedule. To help us do that:

  • Please arrive early to your appointment. 
  • If you’re running late, call us as soon as possible. Arrivals more than 15 mintues late will need to be rescheduled. 

Cancellations & No- Shows

We kindly as for atleat 24-48 hours notice if you need to cancel or reschedule your appointment. We understand that emregencies, family, and career conflicts are unpredictable and may cause you to cancel your appointment. 

Missed appointments or less than 24 hours notice cancellations are subject to a fee. 

Insurance & Payments

Please call our office to find out if we are in network with your insurance plan. While we file all insurance claims for you, your insurnace plan is a contract between you and your insurance company. Please remember you are fully responsible for all fees charged by our office regardless of your insurance coverage. We will make every effort to get your insurance claim paid. 

You will receive a monthly statement until your account balance is paid in full. We appreciate your prompt payment. Please note that balances over 60 days past due will be subject to a finance charge of 1.5% per month (18% annually). If payment is not received within 90 days, the account may be referred to a collection agency.
